When this happens, try the following:
1) close the app that has used the port
2) unplug the OLS
3) plug it back
4) unplug again
5) plug and start using it.
If it works, it means that either the port doesn't get gracefully closed or that it fails on the hardware side. I experienced this behavior when unplugging a device that I was connected to. Had to go trough this steps in order to be able to connect again.
